In Swansea Bay University Health Board, The Care After Death Service is available to support anyone that has been affected by death or bereavement across Swansea and Neath Port Talbot.
The Service offers all kinds of practical advice, information and guidance when a person dies. We are here to support you through your bereavement and we offer support from the initial days following your loss through the weeks and months into the future, should you need it.

9.00am - 5.30pm Monday to Friday, and 9.00am - 12.00pm Saturdays

A referral to the Care After Death Service can be made for any person that has consented and needs support following an expected or unexpected death
The Service offers all kinds of practical advice, information and guidance when a person dies. The Team are bereavement support trained to help each person they speak with get the right support for them (note- this is not a counselling service).
Please ensure you have the consent of the relative/friend before making the referral and make them aware they can expect a telephone call. This Referral will take approximately 5 minutes to complete
