How do I access out patient musculoskeletal physiotherapy?
Referrals for Out Patient Physiotherapy  

Referrals for outpatient physiotherapy services can be done via: 

  • Self-referral - online booking, telephone or paper self-referral forms.
  • Direct referral from GP Surgery, Hospital Consultants or other health care professionals.

Self Referral via Online Booking or Form:

You can refer yourself for physio by using the online booking system or filling in one of the forms below:

Physiotherapy are running a Physio Access Clinic at the Resource Centre to which patients can refer themselves into by booking an appointment in advance.

The easiest way to book an appointment is by using the following link :

The appointments are uploaded 48 hours in advance. Alternatively, appointments can be booked via telephone on 01792 487453 between 9.00am and 10.00am Monday to Friday excluding Bank Holidays.

Once you have completed the form, it can be posted, to physiotherapy department at either Neath Port Talbot, Singleton or Morriston Hospitals, where you will then be put on the waiting list.

There are two ways you can access the service in Carmarthenshire.


You can be referred for physiotherapy by a doctor, consultant or other healthcare professional (eg nurse practitioner, podiatrist, occupational therapist or physiotherapist)

Once they have received a referral from your doctor, consultant or healthcare professional, a senior member of the team will then decide if the referral is urgent or routine based on the information provided. We aim to see urgent referrals within two weeks and routine referrals within 14 weeks of receiving the form.
